Second fatality at Gruver grain facility

An investigation is underway after a man falls to his death inside of a grain elevator. This is the second time in 4 years that an employee has died at Sunray Co-op.

According to Occupational Safety and Health Administration this is the third incident at the Gruver facility since 2008. Twenty six grain handling fatalities have been reported by OSHA across the country in 2010. They are currently investigating the Gruver incident.

Gruver police officials confirmed a fatality at the Sunray Co-op facility, but would not release any other information.

OSHA officials told me representatives are currently in Gruver but could not release any other information until the investigation is complete.

According to reports the grain facility in Gruver was cited in 2008 after a fatality and again after complaints in 2011. Sunray co-op has not released a statement at this time.

I spoke with Emily Thompson, who lost her son Heath Grimes in the 2008 accident. She contacted us after she and her daughter heard about the latest death at Sunray.

"She said it brought it all back again. Just brought it all back. And it did me too. I mean it's like 4 years have gone by and now 2 deaths there. That just don't make sense," Thompson said.

Sunray Co-op was cited with 8 serious violations and fined $12,000 in 2008 according to reports by OSHA.

Thompson says her son never received safety training.
